Become a DoctorCare Patient
The process for enrolling new patients involves several steps:
- Open and print out the new patient forms then fill them out legibly and
fax (480-575-0512) back or email a scanned copy. Please include copies of insurance cards and social security number. - Please send us an email to confirm that we received the fax of
your completed forms. - Doctorcare Staff must verify insurance eligibility -- a process that can take as long as a few days.
- An appointment will then be set up when the doctor or NP is in your area.
